Template for PSBs to Create End-User Request Form

An application form for the end-user needs to be completed in cases where an application is being made to a Public Service Body (PSB) for access to an Anonymised Microdata File (AMF).

All requests for access to an AMF will be received and reviewed by PSBs who wish to share data. As a support to PSBs, the CSO is providing a template for PSBs to create the end-user AMF request form 'Template for PSBs to Create End-User AMF Request Form'. The PSB may choose to edit this template so that it is tailored to their specific needs. PSBs will have very limited decision-making powers in relation to subsequent requests, since approvals must be non-discriminatory under the DGA once access is initially granted to an AMF. The PSB will provide a copy of the anonymised dataset to all successful applicants and may stipulate conditions on security and data destruction in the AMF Request Form.

This template may be used where an individual or entity (end user) wishes to apply for access to an AMF from a PSB. In cases where the end user wishes to apply to the PSB for access to a dataset which contains protected (including confidential or personal) data, they may do so using the template available from the Office of the Government Chief Information Officer’s website:

AMFs contain microdata that are provided only in such a form that the information related to an identifiable entity/person cannot be directly or indirectly identified. They contain no protected data and therefore are not subject to the General Data Protection Regulation.
