DOCTOR: During the past 12 months, how many times have you consulted with a General Practitioner (GP) in relation to your own health?

A health consultation with a Pharmacist does not include:

NURSE: During the past 12 months, how many times have you consulted with a Community/Public Health Nurse in relation to your own health?

Attendance at hospital for blood tests and other routine diagnostic tests not requiring admission are also excluded.
