    DOCTOR: During the past 12 months, how many times have you consulted with a General Practitioner (GP) in relation to your own health?

    Note: Consultation with a GP should be on a respondent’s own behalf only, and may take place in GP surgery, at patient’s home or by telephone. Respondent should count all consultations even if they are repeat visits for the same condition. One exclusion exists whereby phone calls for test results should not be counted as a consultation.

    PHARM: During the past 12 months, how many times have you consulted with a Pharmacist in relation to your own health?

    Note: Consultation with a Pharmacist refers to the provision of health advice to the respondent (e.g. advice on the cause of symptoms and suitable treatment or advice regarding the potential effects of a medication) and includes:
    -Attendance for prescription or over-the-counter medications and medical devices (e.g. dressings, home health test kits etc.) where health advice was received
    -Attendance for health advice where no medications or medical devices were received
    -Attendance for health testing/screening where advice was received.
    A health consultation with a Pharmacist does not include:
    -Attendance at a pharmacy or other retail outlet to purchase over-the-counter medication without the receipt of health advice(i.e. simply picking a product from a shelf and paying for it at a register).
    -Attendance at a pharmacy or other retail outlet to purchase non-medical products,( e.g. hair dye, toiletries, electrical devices).
    -A consultation that fulfils the description above, but where the advice was received for another individual.

    NURSE: During the past 12 months, how many times have you consulted with a Community/Public Health Nurse in relation to your own health?

    Note: Consultation with a Community/Public Health Nurse should be on a respondent’s own behalf only, and may take place in surgery, at patient’s home or by telephone. Respondent should count all consultations even if they are repeat visits for the same condition. One exclusion exists whereby phone calls for test results should not be counted as a consultation.

    OTHERAPP: During the past 12 months, how many Other Health Consultations did you have?

    Note: 'Other Health Consultations' includes consultations with the following:
    - physiotherapists, -occupational therapists, -dieticians, -speech and language therapists, -home helps,
    -social workers, -community welfare officers, -counsellors/psychologists/psychotherapists.
    'Other Health Consultations' does not include consultations with the following:
    -complementary or alternative therapists (e.g. homeopath, acupuncturist, herbalist, faith healer or other alternative medicine practitioners)

    A&E: During the past 12 months, how many times have you attended an Accident and Emergency (A&E) department in relation to your own health?

    OUTPAT: During the past 12 months, how many times have you attended a medical or surgical specialist as an out-patient in relation to your own health?

    Note: An out-patient is a person who attends an out-patient clinic supervised by a medical or surgical specialist/consultant for the purposes of consultation, investigation and/or minor treatment. Out-patient attendances are almost always based on referral by a General Practitioner (GP). Visits to psychiatry and antenatal clinics are included as out-patient attendances. Visits to clinics for physiotherapy, occupational therapy and other allied health professional services should be excluded.

    DAYSER: During the past 12 months, how many times have you been admitted to hospital as a day-patient?

    Note: A day-patient is a person who is admitted to a hospital (usually to a day bed in a day ward) on a planned basis for care and/or treatment which does not require the use of a hospital bed overnight and who is discharged as scheduled. Dialysis patients treated in dedicated dialysis units are excluded from this definition. Attendance at hospital for blood tests and other routine diagnostic tests not requiring admission are also excluded.

    INPAT: During the past 12 months, how many times have you been admitted to a hospital as an in-patient?

    Note: An in-patient is a person who is formally admitted to a hospital bed for treatment and care. Persons admitted to day beds and who do not stay overnight are not classified as in-patients, but should be included in the previous question as day-patients. Hospice stay is included in in-patient hospital admissions but nursing homes and respite care are to be excluded.
